Why a Four 20 Pharma patient card?
The patient card is a document that can be carried along with a current prescription and photo ID. And that is to prove that you can carry and consume medical cannabis. The patient card is by no means an official document and therefore cannot confirm the legality of medical cannabis products. If you don't always want to carry your original prescription with you, the patient card will help you prove your eligibility, at least indicatively, e.g. when visiting a restaurant.
How long is the Four 20 Pharma patient card valid?
The validity of your patient card depends on your prescription. The patient card shows the date of the last prescription as a reference, as only the latest prescription always confirms that medical cannabis may be consumed.
How often can I apply for the Four 20 Pharma patient card?
You can apply for a new patient card from us once a year. To apply for a new card, proceed as you did when you first applied.
How do I apply for the Four 20 Pharma patient card?
You can easily apply for the Four 20 Pharma patient card in four steps.
1. Fill out our online form and confirm our terms of use and privacy policy.
2. Upload your current cannabis recipe to us (alternatively, you can send it to us by email, fax or post).
3. We will check your entries for completeness and issue you with your personal patient card within 14 days.
4. After receiving your patient card, all you have to do is sign it to confirm the information.
From which country can I order the Four 20 Pharma patient card?
The Four 20 Pharma patient card is only available in Germany and is only shipped within Germany.
What happens if I lose my Four 20 Pharma patient card?
In case of loss, please contact us immediately and report the loss of your patient card. With our replacement application, you can then apply for a new patient card free of charge. Please note that Four 20 PHARMA patient cards reported as lost may no longer be used. You are not entitled to receive a (replacement) Four 20 Pharma patient card.

What do I have to present when applying?
To ensure that you are a cannabis patient, we need your latest cannabis prescription to create the patient card. This also serves as a reference for the patient card. We also need your name, address, date of birth, prescriber and which cannabis preparations were prescribed.
